UNITY Update: Form VIII Home Evaluation
As of April 1, 2024 Rule 4-104 (4) will be updated as below. This rule update impacts the Home Evaluation when the field officer selects "Not Recommended" in the Supervision Recommendation UNITY data field.
In the Home Evaluation "Agency Evaluation" UNITY data section, when the Supervision Recommendation selected is "Not Recommended" then the new UNITY data field
"DETAILED JUSTIFICATION IF SUPERVISION NOT RECOMMENDED (include why proposed residence is not safe and/or suitable):"
is presented and is required to be completed.

Overview of Home Evaluation Workflow Approval Paths
The field officer makes the Supervision Recommendation based on the Home Evaluation findings to "Recommend" or "Not Recommend". The ICJO Compact Officer makes the final determination to "Approve" or "Denies" the Home Evaluation Supervision Recommendation.
New Form IX - Failed Supervision
The new Form IX: Failed Supervision Report contains all required information for a Receiving State to submit a failed supervision report to the sending according to Rule 5-103A.
The new form also contains required response fields for the Sending State to indicate the plan of action to return the juvenile or secure an alternative residence.
Form IX – Failed Supervision Report requires the Sending State to either “Return Juvenile” or “Secure Alternative Address” within 10 business day
